Spiritus Zillmere Community Care

We have offered services from Breakfast Creek to the northern suburbs of Brisbane and into the Pine Rivers Shire since 1976.

Our service is partially funded by a variety of programs including Home And Community Care (HACC), the Department of Veterans’ Affairs (DVA nursing and VHC), and the Department of Health and Ageing (CCP and EACH Packages).   There is a small co-payment for some of our services.

How can we help you?

Most of our services are designed to assist older people and young people with disabilities to remain at home. Once you are referred to our service our staff visit you in your home and complete an assessment to identify what assistance is required for you and together with you plan your care. We review the plan of care regularly to ensure that it continues to meet your needs. Our staff aim to provide you with the highest quality health care service to help you enjoy a healthier life.

Quality Assurance

We are fully accredited by the Australian Council of Healthcare Standards and our staff receive ongoing education and professional development to ensure we continue to meet the changing health care needs of our community.

Volunteers

Our service is enhanced by the assistance of our dedicated volunteers, who offer their time and expertise with social support assisting with group activities (outings, day respite), in-home companionship visiting or by providing office-based support in our branch.
